Home Care Packages

Co.As.it is an approved provider of home care packages.  The Australian Government’s Home Care Package (HCP) program helps you access a package of care that’s in line with your needs.  It’s the next level up from CHSP for people with greater needs than the basic entry level.  A package of care helps you to live independently in your home as long as you can.

A care plan helps us identify the services that you need so that you may be as independent as possible.  Once we’ve identified your needs the right services for you can be put in place.

If you already have a package with another service provider you can SWITCH your package to Co.As.It. Community Services.  It’s very easy and we can help you to do this (just call our office).

The home care packages are delivered with your needs in mind and allow you to exercise choice in the way your care services are offered and delivered. 

Care services may include the following but not limited to:

  • social connections
  • domestic assistance
  • personal services
  • nutrition, hydration, meal preparation and dietary services
  • mobility and dexterity services
  • nursing, allied health and therapy services
  • meal preparation
  • mobility assistance and equipment
  • transport and personal assistance
  • continence management
  • counselling
  • dementia support
  • technology help

How to access a care package

My Aged Care will assess your eligibility.  Call them on 1800 200 422 to request a face to face assessment of your needs undertaken by ACAT, in your own home.

When you are approved for a package, My Aged Care will send you a letter to advise. 

When you receive the letter assigning you a home care package you can then request to have Co.As.It deliver your care and My Aged Care will send a referral through to us. 

Our intake officers, Michelle and Enza, can guide and support you to create a care plan that suits your needs.  They will call you to arrange a visit and discuss the services you require.

A Care Services Coordinator is  then assigned and they will be your contact for care delivery.

All levels of Co.As.It. home care packages can cater to the needs of the culturally and linguistically diverse (CALD).

A person on a home care package will be asked to pay a basic daily fee, as well as an income-tested fee, if the person’s income is over a certain amount.  These fees are assessed by Services Australia (a government department) not Co.As.It.

Your package with Co.As.It will not attract a basic daily fee. 

There are 4 levels of care:

LEVEL 1 Basic care needs

LEVEL 2 Low-level care needs

LEVEL 3 Intermediate care needs*

LEVEL 4 High-level care needs*

* Note that Co.As.It does not charge a fee for an initial assessment by a registered nurse
