The Short Term Restorative Care (STRC) is an 8-week program especially designed to help you regain or maintain your wellbeing and independence.
Help to recover from an injury or illness.
The STRC program is government subsidised and it aims to get you moving and retain your independence. Take advantage of specialists’ knowledge to feel strong, healthy and happy again.
STRC is tailored to you and services may include but not limited to:
- occupational therapy
- physiotherapy
- aids and equipment, including mobility aids
- continence management
- nursing support
- nutrition advice
- exercise physiology
- personal care
- podiatry
Goals based 8 week program
We will help you to identify your goals and what you would like to achieve. The 8-week program is delivered in the comfort of your own home or within the community.
You can access 2 episodes of STRC within a 12 month period.
Following the end of the restorative care period, you will be able to continue some services if you are assessed as having a need to support you in living in your own home. Co.As.It. will be able to guide you in accessing further services to meet your goals.
The multidisciplinary team of allied health professionals, including your GP, will help you improve your mobility, reduce pain and improve your overall health.
Eligibility
To access STRC you must have undergone an assessment by the ACAT and approved for the STRC package for 8 weeks.
ACAT will conduct a comprehensive assessment of your physical, medical psychological, cultural, social and restorative dimensions of your care needs and provide a choice of appropriate services to meet your needs.
